Online Referral - to refer to a specialist practicing out of one of our sites, complete the secure form on the right, and submit directly to Eye Surgery Associates (ESA).

Oculo - optometrists may submit a referral to one of the specialists working out of our premises via oculo.

Referral Form - download an A5 ESA Referral Sheet (PDF), complete and forward to ESA.  To request referral pads & cards, please email Optometry Name, Optometry Address to marketing@eyesurgery.com.au

Secure Electronic Messaging - ESA provides the specialists who practice from our premises with practice management software that communicates with Argus and HealthLink.

Urgent Referral

When referring a patient urgently to a specialist who practices from an Eye Surgery Associates site, please call one of our site locations directly. Mail or fax may be used for non-urgent referrals. When you call the specialist’s rooms, our reception staff will endeavor to book your patient in with an appropriate doctor in the time frame that you specify. If an appointment is not available due to fully booked consulting lists, the receptionist will liaise with an orthoptist or doctor to see if it is possible to overbook a session. For after-hours urgent ophthalmic care, please direct your patient to the Royal Victorian Eye and Ear Hospital (RVEEH) Emergency Department.

Referral via Oculo

Online Referral to refer to a specialist practicing out of one of our sites, complete the secure form on the right, and submit directly to Eye Surgery Associates (ESA). Optometrists may submit a referral to one of the specialists working out of our premises via OCULO, HealthLink or Argus. ESA provides the specialists who practice from our premises with practice management software that communicates with Argus and HealthLink.
